After you have scheduled your first appointment at Baus Family Dental, please take a moment and complete the following required medical forms. You can complete these forms when you come to our office. However, it may be easier for you to do this in advance and allow us to see you more quickly when you get to our office! You can download the documents, print, and bring these with you to your first appointment.

Please remember to include information such as any diseases, medications, allergies, joint replacements, or any other medical issues. These are important to us in treating you and your total health. Thank you!
